The first one is for the calibration temperature of your hydrometer. I believe that some are calibrated at 60 F, and some at 68 F, but check your particular one to see.

The second one is for the temp of your must when you took your OG, or Original Gravity / Starting Gravity reading

The third one is for the temperature of your must when you took you FG or Final Gravity reading.
