Difference between refractometer and hydrometer

I have a 5gal batch of concord wine on the go. Today I took all the skins/pulp out and hand squeezed the mesh bag to get as much juice as I could.
It's been fermenting about 4 days.

After,I took a couple drips of juice and it read 1.055 on refractometer. On the Hydrometer, it read 1.046.
Any thoughts on why I had such a large difference? I checked both with some plain water and they both read 1.00
Initial SG was 1.080
