just scanning through some posts I have come across the phrase "pitch yeast". What does this mean and when or why is yeast pitched?

Nothing magical about it, it just means putting the yeast into the must when the time comes to start the fermentation

Yep, unless one makes a yeast starter/hydrator, it is typically just sprinkled (pitched) right on top of the must and not stirred in.
